Bodies on top of bodies, IV's on top of IV's
A rather holocaustaul image, there was so much killing that corpses were everywhere, piled on top of each other, and the hospitals were packed with the wounded and dying.
Double entendre of the second line:
IV is the Roman numeral for 4, thus Kendrick is saying that there were .44’s everywhere.
IV is the acronym for the intravenous drip inserted into many hospital patients' arms to get liquid directly into the bloodstream. Gunshot victims would almost certainly be given blood transfusions by this method.
Note – If used in another context, the intravenous needles lying in piles could be those from addicts shooting up heroin and other drugs in the streets.
