It's a bath house for brave soldiers

These junkies, ironically referred to as “brave soldiers” are given sedatives by the staff at the rehab center. The image of the drugs placed on their pillows is an allusion to fancy hotels that put little mints and chocolates on bed pillows in the rooms. Kind of an ironic image. Props Aes!
The imagining of fucking the female staff is just their sadistic thoughts they use to pass the time in the absence of their former vices.
