Aesop only later struggled with mental illness, but this line shows he already understood its challenges.
The most widely used (and “credible”) treatments for mental illness (or “crazer-antidotes”) are psychiatric drugs, which can have severe side-effects. If Mary Poppins was around with a “spoonful of sugar [to help] the medicine go down”, the medicine’s effects are so “bitter” you would need to make sure not to spill any sugar as you would need all of it.
One of the things that make this line striking is the contrast between the childish image of Mary Poppins and the very dark issue of mental illness.
