When someone gets shot the bullets are usually made of led which can often poison a person
This can have another meaning as well. If someone in the hood gets shot, those that “survive” the victim are influenced to retaliate due to their peers believing that it’s weak or it’s justice if they kill the person who murdered the first person. This in turn keeps a deadly cycle going which is poison in thought. A back and forth between the two parties instead of letting God (or Karma, whichever you believe in) take care of it. What goes around comes around but if those involved send it back, it becomes a game of hot potato. Ultimate poison, in the end the whole hood gets killed off over nonsense.
The “niggas” in the hood are playing roles of doctors. The same way doctors give shots, the niggas do too, both are Poison. That’s why the very next line Nas says hospital, to continue the entendre.
