In others words she’s bright “like a light-bulb”. 45 refers to the wattage of the bulb.
Also a reference to a .45 caliber handgun, which is fitting with the following line about how he “could’ve died” but kept going (deciding not to pull the trigger).
“I could’ve died” refers to his attempted suicide.
“After I came off tour, we went to Australia and I was just super depressed. I mean, I tried to kill myself”

This also relates to the BtI screenplay, when The Boy goes to his mansion and sees four dudes outside his front door waiting for him with “45s.”
