This is Martha telling Roman to take some meds and calm down. the line “Some alterations on your clothes and your brain” is Martha saying to Roman (who is gay and dresses flamboyantly) that his brain is just broken and needs to be fixed. The whole hook is also a satire on the Catholic Church’s stance on homosexuality. In her Grammy performance, Nicki made it evident that Martha was trying to “fix” Roman by having him sent to a priest to be exorcised, kind of like how some parents send their kids to camps to “pray the gay away”.
