The ACL is the Anterior Cruciate Ligament of the knee. Fat Joe employs an extended, slightly belaboured, metaphor: From the previous line, he sees the bedroom as a sort of basketball court in which he is dominant, and his performance outstanding.

“Back-shots” (here, a double entendre) are shots that bounce of the back-board as they are scored. ACL injuries often occur “when an athlete rapidly decelerates, followed by a sharp or sudden change in direction”*. We are then left to imagine that Fat Joe somehow manages to achieve, while in bed with a girl, the injury-inducing intensity of “going hard in the paint”.
