The toxic cloud is referring to all the smoke that was present during the tragedy, which seems pretty obvious, but even with that there, you could still see the people falling from the windows, as if they looked like debris. Powerful line.

He’s also saying that the “fallout” from this tragedy goes beyond either the physical fallout (smoke & debris) or the people falling out of the buildings–there’s going to be an immense emotional fallout for the survivors and their friends and families as well. As well as the political political fallout, i.e. the wars in Afghanistan, Iraq, and the “war” on the abstract noun terror.
