He meets with a dealer in a subway station (or on a subway train) and buys a shot of heroin. Afterwards, he rests on a used, dirty mattress outside. (Probably in a back alley somewhere.) The “throb bloating” probably comes from a tourniquet used to see his veins/entry points, but it may also come from the injection causing his veins to pop out.

The irony of America, and Holy Wood – Manson’s satire of America – is the things it’s citizens acts like they hate the most are really the things they’re most entertained by: death, sex and violence.

When you put all those together, you get America (Holy Wood) as we know it, and the bitter, narcissistic offspring it creates, like, for example, Holy Wood’s protagonist, Adam, or Manson himself.

Ray moves through time (grows up) in a Washington D.C. ghetto, where he navigates his way around the racist cops who’ll try to abuse him or lock him up, and the thugs who will try to rob or, worse yet, kill him. The history of African Americans still affects both groups: the cops grow up racist due to their upbringing and culture, and the thugs feel conditioned to fail and stay in the ghetto where they resort to crime to sustain themselves, often winding up in prison.
