“Came in with scrubs” is a play on words. Scrubs are the light blue outfit worn by doctors and nurses; however ‘Scrubs’ is an insulting moniker for weak competition.

“Lost my patience” is an example of Em’s use of play on words. It could also be perceived as “lost my patients.” Sedation is a practice that doctors used on patients to put them to sleep before surgery. Eminem is saying that his patients woke up from their surgery and ran away.

“Operation” can refer to an actual medical procedure, and if Dre were a real doctor that’s what he would be doing. But in this context “operation” refers to a business – Aftermath Entertainment – that only Em and Dre are capable of running.

Kendrick Lamar who is currently considered one of the greats to come out of the new school is also signed to Aftermath so Em may also be referencing Kendrick helping to run the game as much as he is talking about himself.

“Lean” is slang for codeine, aka purple drank. It’s a popular drink amongst rappers, especially southern ones. And the Leaning Tower of Pisa in Italy, as you can tell by the name, is slanted because it was built on a weak location.

He pronounces “Pisa” as “Pizza”, thus topping it off with a play on mushrooms, which is also a type of drug. And he makes all of sound like olive, another pizza topping.

Odd Future is a hip-hop clique led by Tyler The Creator and other talented rappers in their early 20s that enjoy making jokes about anal rape. I tried suicide once and i’ll try it again
that’s why I write songs where I die at the end

Seeing as Eminem has died in his raps before, from 1997’s “Murder Murder” to 2013’s “Bad Guy” – he tends to rap for as long as possible because he wants to get it all out before he “dies” in the song.

Bonnie & Clyde were a crime couple who where shot up in their car. Ronnie is Eminem’s uncle who committed suicide. Ronnie being behind the wheels suggests that Em is already dead by this point. Arkham Asylum is a fictional mental hospital from the DC universe, tying back to the Batman reference.
