Houston Hip Hop is famous for promoting lean, an intoxicating drink made with prescription strength cough syrup containing Promethazine and codeine, accompanied by a soft drink and/or hard candies (Primarily Jolly Ranchers).

The line could also be referring to the “gangsta lean,” a classic driving pose first named in song by William DeVaughn. This defiant posture of laidback invincibility was also famously referenced by Snoop Dogg, Clipse, Joe Budden and many others.
