These are extreme opposites.
If you were to be treated like a god that would be to be treated extremely well.
To be treated like a leper is terrible. Leprosy, also known as Hansen’s disease, is a chronic infection caused by the bacteria Mycobacterium leprae and Mycobacterium lepromatosis. If you are a leper no one wants to be even be near you because it is disgusting and slightly contagious.
Those who appreciate hip hop and recognize his talent/place in hip hop culture treat him as a God. People who are not intimately familiar with his profession and his place in rap culture only see the bummy quality of his appearance/person.

Through the dark ages and into the mid 19th Century the world was largely ruled by royalty (The days of Kings and Queens) this royalty appointed musicians to entertain them in a similar purpose to what jester did, entertain. Now he and other rappers are stuck between to extremes they shift between. Rappers are either despised by parents and Middle Americans. This he equates to the treatment of lepers, a class of people exiled for their condition and despised for it. The other extreme is that of worship, Children and Listeners revere Atmosphere. They treat him with the reverence associated with royalty and divinity. Neither of these are maintainable ways to live, so he has to find a middle ground
