A Chuck Wagon was a wagon used to carry food and supplies while the settlers were traveling west. By this Pusha is referring to how his car is always carrying the goods(cocaine). Most of these wagons were heavy, so 12 horses would be enough to pull the wagon.

The Amish, being a pretty devout sect of Christianity, often use biblical names for their children; Jebediah and Ezekiel are prime examples of such.
The original song starts with “as I walk through The Valley of the shadow of death” which is a reference to Psalm 23:4, a part of the book Ezekiel. This may be why Ezekiel was chosen here.
This could also be a reference to the Biblical book of Ezekiel which, along with Isaiah and Revelation, is infamous among religious scholars for being filled with vague poetry that is open to interpretation. As a member of the religious Amish community, he’d know about this book. Considering how irreverent he is the music video, though, it’s likely he just thinks he’s crazy, amplifying the statement “Even Ezekiel thinks that my mind is gone!”
