Known as coulrophobia, a fear of clowns. It’s also a double entendre referring to his distaste for those in the rap game, that are gimickly playing along with the show instead of innovating the world.

Aesop asks himself whether he is truly human or a lizard (i.e. cold-blooded)
Also the word mirror implies something visual to emphasize the wordplay between pictures and pitchers. It could mean Aesop is trying to get the listeners to see what he sees, which is a hard task (again a headache and a burden).
He’s literally feeding and teaching hungry drunks with his lyrical rhyme-style.
The “Simian or lizard” line is a reference to neuroscience, I think. The reptillian part of our brains' early development that governs base emotions or the “simian” parts of our brain that govern more complex things like language and perception.
Aes' is trying to figure out if his feelings are based on his most primitive fears or his more refined ideas about life and his place in it. But there’s “a disappearing difference”, all that matters is how he feels.
Possibly the simian represents that which is higher and more evolved than the primitive lizard; he wonders whether he is actually destined for greatness (simian) or to fail (lizard)

it relates to the last line; his head hurts, he’s presumably been vomiting, staring at himself in the mirror, sweating “pitchers.” just sweating a lot.

Possibly the “sweating” of “pitchers” is the relentless push to join the dark side that inevitably occurs upon gaining fame, and proving ingeniousness. He is in the mirror wondering how well he is doing in the fight against these evil propositions. simian: innocent, genuine. lizard: selfish, evil intent.
Aes is standing in front of the mirror, possibly hung over, mentally anguishing whether or not the man staring back is human (simian) on the inside, or “lizard brained” so to speak (cold, unrepentant, etc).
