Literally, internal bleeding is bleeding which occurs inside the body. Here, Cudi is not referring to literal internal bleeding but rather the “bleeding” caused by heartache. He has “Tried it all” to stop this bleeding" but unlike a cut, there is no way to stop it.
By “leaking out”, Cudi is referring to how he is pouring his heart into his songs, and that he can’t contain his emotions inside.

The Tuskegee syphilis experiment was an infamous medical experiment which took place in the U.S between 1932 and 1972. The experiment involved studying the natural progression of syphilis on rural African Americans, who were told they were receiving free health care, and being cured of the disease.
“Air men” is a reference to the Tuskegee Airmen, who were a group of African-American pilots who fought in World War II.
Here Omen is reminding us of the fact that in the past African American men have taken advantage of by the Government. You can call him a creepy conspiracy theorist, but he will not let the same thing happen to him.

Omen is referring to the overdramatic, and ridiculous pandemonium that occurs when the public hears of a new sensationalised flu outbreak.
People become so paranoid that they think if someone coughs or sneezes near them, they will die.
This line seems to be an earlier version of “wipe his nose” (a popular idiom in hip-hop after artists like DaBaby and Roddy Ricch began using it in mainstream music) before the public eye caught wind of this signal to “kill someone.” Wiping your nose, being a common gesture after a “sneeze” or “cough” as the line suggests.
