Eminem is really mad sometimes and talking and rhyming much shit sometimes while he’s dissing many other people (most singers which he folds and blames in his way) …
This line and the one above it directly reference Alfred Hitchcock’s “Psycho”, particularly the iconic line, “We all go a little mad sometimes.”
I think he means this as a “comeback” for his verse in his song, not really now! He could also mean that he is back with his choppers and rappers on a song
His fans are everywhere, they makin' him crazy because they ask so much about him and his rapping skills! Then he tells them that he is only a black “nigga”…
