Meaning

His boy is so mashed up he can’t stand up straight so he has to pick him up set him down and give him some water or splash cold water in his face, then give him some Vicodin to calm him down.

This is Kendrick’s introduction to the theme of his generation being drug-oriented in regards to both medical and recreational drug use. This is perhaps his most extreme example: a friend is having a bad experience with some sort drug and Kendrick’s idea of how to “take the pain away” is to bring him MORE DRUGS (Vicodin).

“I set him in” sounds exactly like “acetamin”* which is short for acetaminophen. Acetaminophen (Tylenol) is a common pain killer, so it works with the “take the pain away” bar. Basically, his friend’s headaches are kickin' in, and Kendrick’s trying to help him out. He gives him Tylenol at first, but the pain is too much. Almost. Acetaminophen is almost always found mixed with hydrocodone in Vicodin pills. So he gives him the stronger painkiller, Vicodin. This is also part of the theme of tolerance; weak pain killers aren’t enough for his niggas, they need the strong shit to ease their pain.

“Cold water” is a reference to Cold Water Extraction, a simple do-it-at-home method of extracting ingredients such as acetaminophen (Tylenol) that produce no high leaving only the desired drugs, which in this case is hydrocodone (Vicodin). This is done for the fact that acetaminophen is bad especially in large or repeated doses, as well as that it mixes very bad with liquor (up to 48 hours after acetaminophen use, though some people argue up to 3 weeks)
