He’s referring to the Kama Sutra an ancient indian hindu text on human sexual behavior (revised with graphic depictions of different sex positions) Kama Sutra
This line is a double entendre, not only is he saying fuck your position in regards to the rank one might hold but also fuck your your position in referring to said (sexual) text making his insult more “intimate/personal”. And then to “hold” that position meaning he wants that insult to stick to your rank, the sexual side again making it a more intimate insult in that “holding” that “position” would make it uncomfortable for said person and possibly a public/witnessed affair.
The initial “I do it big” means all of the actions in the ensuing lines (above⁾ would be done in a most grandiose way
